We use a state-of-the-art Electronic Medical Records (EMR) system that gives our patients 24/7 access to their records through the online patient portal, and our physicians practice evidence-based medicine and focus on managing chronic diseases to help our patients reach their personal health goals.

Griffin Faculty Physicians Primary Care Offices, recognized for providing the best Patient-Centered Care, have all achieved top-tier recognition from The National Committee for Quality Assurance for using proven, patient-centered processes that focus on coordinated care and patient-doctor partnership (this includes Ansonia, Family Healthcare, Oxford, Seymour, Shelton, Naugatuck, and Southford).

All offices are recognized as Patient-Centered Medical Homes - a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. These programs are recognized by the Centers for Medicare and Medicaid Services (CMS) and the National Committee for Quality Assurance (NCQA), and GFP's additional offices are expected to soon follow suit.

Griffin Faculty Physicians earned the top-tier Level 3 Patient-Centered Medical Home Recognition for achieving all of critical elements of high-quality, patient-centered care, including access, patient self-management and support, test tracking, advanced electronic communication and performance reporting and improvement. NCQA standards aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.
