Our Primary Care Office is conveniently located across the street from Griffin Hospital’s Emergency Department at 67 Maple Avenue. Parking is available behind the building, and the entrance is located around the front.

From Rt. 8 North
Take Exit 16 to merge onto Pershing Dr toward Ansonia. Turn left onto Division St. Take the 3rd left onto Maple Ave. The office will be on the left.
