Griffin Faculty Physicians Primary Care Offices, recognized for providing the best Patient-Centered Care, has achieved top-tier recognition for using proven, patient-centered processes that focus on coordinated care and patient-doctor partnership from The National Committee for Quality Assurance for our Offices in Shelton, Derby, Seymour and Oxford.
These offices are now recognized as Patient-Centered Medical Home – a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. These programs are recognized by the Centers for Medicare and Medicaid Services (CMS) and the National Committee for Quality Assurance (NCQA), and GFP’s additional offices are expected to soon follow suit.
Griffin Faculty Physicians earned the top-tier Level 3 Patient-Centered Medical Home Recognition for achieving all of critical elements of high-quality, patient-centered care, including access, patient self-management and support, test tracking, advanced electronic communication and performance reporting and improvement. NCQA standards aligned with the joint principles of the Patient-Centered Medical Home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.

When will the transformation be complete?
The process of being recognized by NCQA as an Advanced Primary Care Practice/Patient Centered Medical Home is a lengthy one. It involves changes in policies, practices and systems. We have already begun the process and have key infrastructure already in place, including an Electronic Health Record (EHR) System in place which includes a patient portal with 24/7 access for our patients. Other changes are in progress.
